2. Provide the following information:

• EIN/TIN number
• Business Street Adress
• Minutes showing officers appointed to conduct business
• Corporate By-laws or Partnership Agreement

3. Each Signer on the account must have:

• Driver’s License with current street address
• Current Major Credit Card or other Identification.
• Social Security Number
• Home Phone Number
• Employer
• Employer Phone Number
• Title or Capacity serving on the account
